If it is necessary for an elder to relocate for assistance, Senior Transitions is knowledgeable about matching your loved one’s needs with the right location and knows the local options very well. Senior Transitions Geriatric Care Managers will work with you to explore financial resources available to pay future care expenses, explain what assistance is covered by governmental agencies, such as Medicare or Medicaid, help evaluate residential living choices such as Assisted Living vs. Long Term Care. Your Care Manager will also arrange for tours of facilities and assist with questions to ask the staff.

If a new living location is selected, Senior Transitions is able to assist with making the transition from home smooth. They will also monitor the senior’s adjustment to the new facility and they will also be available to you for ongoing concerns as you require.
