Jen McConnaughhay, MSW
Jennifer McConnaughhay moved to Florida from Virginia to complete her college education. She earned a Bachelor's in Social Work in 2006 and her Master's in Social Work in 2007 from Florida State University. She has worked with the elderly since prior to college. She loves the career path she has chosen and views her work as a privilege. "Our seniors are so important and yet often times they are the forgotten population." Jennifer discovered her passion when she lived with her grandparents for two years. Before joining Senior Transitions she served as the social service director at a long-term care facility. She has also worked and volunteered for hospice. Jennifer is very familiar with local resources available to seniors. This is through experience with case management and discharge planning at a rehabilitation hospital. Jennifer is married to Allen and they have one daughter, Emma Kate and another girl on the way.
As a member of the Senior Transitions team, Jennifer provides case management to elders and their families at home or in facilities. She works closely with health care professionals to ensure that their loved ones receive optimal care. Jennifer attends care plan meetings and appointments with patients and family members. She visits patients weekly and updates families of any changes.
